    Student Medical Conditions

    In order to fully support you during your visit at ENBS, the following questions ask for further details about your medical history. Please provide any details you feel are relevant and are comfortable with disclosing at this time.

    At the English National Ballet School we are committed to developing an inclusive and supportive environment where all students have the opportunity to fulfil their potential. We value the positive impact that diversity has in our school,and actively seek to attract applications from people from diverse backgrounds. To help us develop our widening participation data, please complete the questions below. The information you provide is strictly private and confidential. Completion of this form is entirely voluntary, and you can select "prefer not to say" if you would rather not answer any particular question.
    Anyone who spent time in care as a child (i.e. under the age of 18). This care would have been approved by the state through a court order or on a voluntary basis. It can range from as little as a few months to as long as ones whole childhood (18 years). Such care could be in foster care, residential care (mainly children’s homes) or other arrangements outside the immediate or extended family.
    A young carer is someone aged 25 and under who cares for a friend or family member who, due to illness, disability, a mental health problem or an addiction, cannot cope without their support.
    You should have had no contact with your parent/guardian for at least 12 months before the start of the course, and be unlikely to have a relationship with them again.
    Individuals who identify as refuges have typically been forced to leave their country in order to escape war, persecution, or natural disaster. An asylum seeker is currently in process of being granted their refugee status.
